I can't enable afib notifications as that menu option is missing from the app. Why is this? And how can I enable it? I have looked in All settings>Health Features and afib is not there at all.

It seems there's a bit of confusion regarding the functionalities of the ScanWatch 2. Unlike its predecessor (ScanWatch), the ScanWatch 2 does not automatically monitor for signs of AFib.
However, it is equipped with the capability to record a clinically validated electrocardiogram (ECG) and detect signs of atrial fibrillation during your on-demand ECG measurements.
To utilize this feature, please ensure that you initiate a manual ECG recording.
