1- Agree to share data with Heartbeat Health

By checking the box, you electronically authorize Withings to share personal data with our partner, Heartbeat Health. Personal data includes:

• Name
• Date of birth
• E-mail address
• Residence state in the US
• Your first ECG trace for review

2- Agree to be contacted by text message by Heartbeat Health (State dependant)

By checking the box, you electronically authorize Withings and our partner, Heartbeat Health, to text you at the number you will provide us about the ECG functionality on the watch, and other information that may interest you. Consent is required only in order to activate the ECG functionality on your device.
