1- Agree to share data with Heartbeat Health

By checking the box, you electronically authorize Withings to share personal data with our partner, Heartbeat Health. Personal data includes:

• Name
• Date of birth
• E-mail address
• Residence state in the US
• Your first ECG trace for review

2- Agree to be contacted by text message by Heartbeat Health (State dependant)

By checking the box, you electronically authorize us and our partner Heartbeat Health, to text you at the number you provided us in connection with the activation of the ECG functionality on the scale, and with other information or programs that may interest you. Message and data rates may apply.
